After you have worked 3 FULL calendar months (not including the month you started) you will be eligible for benefits on the 1st day of the following month. (Example: Hired: May 27th, 2023; Eligibility: September 1st, 2023). Once you become eligible, please complete and submit the Registration Form to the PBAS Benefits office.

Your 2-year anniversary comes from the first date of purchase. For example, if you received an eye exam or purchased prescription glasses or contacts on June 18, 2023, your vision coverage benefit will renew on June 18, 2025.

If you need to confirm your anniversary date for glasses, contact the PBAS Benefits office.

Please note: The Union office does not process claims or have access to your benefit records.

All claims must be sent to the PBAS Benefits office by either email, fax or by mail. Please submit all receipts when submitting your claim.

Please present your benefit card to the dentist for direct billing. The dental office can contact our office for a breakdown of your dental coverage.

Yes, you will receive your Benefit card after submitting your completed and signed Registration Form to the PBAS Benefits office by email, mail or by fax.

You will need to complete a new Registration Form to make changes and update any personal information. Proof of change may be required. Members must submit a Registration Form to:

·        Add/remove dependant(s) from their plan
·        Change SIN (** proof required)
·        Change address (** members must update their new address and phone number with the Employer,             Local 247 Office, and PBAS Benefits office)
·        Change of name (**proof required)
·        Change of beneficiary

No, health coverage is not provided for spouse or dependant(s).

Full-time members and their spouses and eligible dependant(s) will have dental coverage on the full-time plan (Group B).

Complete the Missing Hours Form to remain in benefit while on leave. The Form must be completed by the member and their employer. Types of leave covered by the plan include compassionate, vacation, illness or injury, parental/maternal, and other employer-approved leaves. Contact the PBAS Benefits office if you have questions.
